Frontier Health consumers have broad access to a system of high quality behavioral health, mental health, substance abuse, developmental disabilities, and vocational rehabilitation services in an integrated system of settings, services, health and behavioral health care practitioners and care levels.

   The continuum operates from pre-discharge screening through discharge of the patient providing assessment of on-going needs and coordinating care within Frontier Health’s scope of services and with agents, organizations and institutions external to Frontier Health.

   Coordination of care maximizes a consumer’s access to appropriate care, choice of available care, transition through levels of care and effective referral and discharge planning and communication.
